Introduction
BioX Unlimited LLC, doing business as Function Bone Health and operating Strength Lab Plus wellness studios, is committed to respecting the rights and dignity of every client. This document outlines your rights when using our wellness education and screening services.
Important: Function Bone Health is a wellness education and screening center. Our clients are not patients. We do not provide medical services, and no healthcare provider-patient relationship exists between you and our Company. The rights outlined below reflect our commitment to treating all clients with respect, transparency, and fairness.
1. Right to Information
You have the right to:
- Receive clear, accurate, and understandable information about our services before booking or purchasing
- Know the qualifications and roles of staff members who will be providing your services
- Understand the nature, purpose, and limitations of any screening assessment before it is performed
- Receive complete and understandable explanations of your screening results
- Be informed of all costs and fees before services are rendered
- Receive copies of all applicable Legal Documents (Terms of Service, Privacy Policy, Disclaimer, Refund Policy, HIPAA-Aligned Notice, Cookie Policy, Do Not Sell My Information Policy)
2. Right to Privacy and Confidentiality
You have the right to:
- Have your personal information and screening data treated as confidential
- Expect that your information will only be used and disclosed as described in our Privacy Policy and HIPAA-Aligned Notice
- Request access to, correction of, or deletion of your personal information
- Know who has access to your information
- Request restrictions on how your information is used or disclosed
- Request that we communicate with you through specific channels or at specific locations
- Receive notification if a breach of your unsecured information occurs
3. Right to Informed Consent
You have the right to:
- Receive clear explanations of all services before they are performed
- Understand the potential benefits and limitations of any screening technology
- Give voluntary and informed consent before any screening assessment
- Withdraw consent at any time, including during a screening session
- Refuse any service without penalty or discrimination (applicable deposits and cancellation policies still apply)
- Receive a copy of any consent form or agreement you sign
4. Right to Quality Service
You have the right to:
- Receive services in a clean, safe, and professional environment
- Expect competent and respectful service from all staff members
- Expect that screening equipment is properly maintained and calibrated
- Receive timely service and reasonable wait times
- Expect that staff members act within their scope as wellness coaches and educators
5. Right to Respect and Dignity
You have the right to:
- Be treated with courtesy, respect, and dignity at all times, regardless of race, color, national origin, sex, age, disability, religion, sexual orientation, gender identity, veteran status, genetic information, or any other protected status
- Receive services in an environment free from discrimination, harassment, and intimidation
- Have your cultural, spiritual, and personal values respected
- Receive services in a manner that protects your modesty and personal dignity
- Communicate in your preferred language (reasonable accommodations will be made)
6. Right to Safety
You have the right to:
- Receive services using safe, FDA-cleared screening technology
- Be informed of any known risks associated with screening procedures
- Expect that our facilities comply with applicable safety regulations
- Report any safety concerns without fear of retaliation
- Receive clear guidance on when to seek emergency medical care
7. Right to Voice Concerns and Complaints
You have the right to:
- Express concerns, provide feedback, or file complaints about any aspect of our services
- Expect that your complaint will be reviewed and addressed in a timely manner
- Receive a response to formal complaints within thirty (30) days
- Exercise your rights without fear of retaliation, penalty, or reduction in service quality

- Share your screening reports with any healthcare provider, specialist, or advisor
- Seek second opinions or additional evaluations from independent medical professionals
- Make your own health decisions based on the totality of information available to you, including but not limited to our screening results and the advice of your healthcare providers
We strongly encourage all clients to share screening results with their licensed healthcare professional for clinical interpretation and guidance.
10. Client Responsibilities
While we are committed to upholding your rights, we also ask that you:
- Provide accurate and complete information when requested
- Treat our staff, other clients, and our facilities with courtesy and respect
- Arrive on time for scheduled appointments
- Follow any reasonable instructions provided by staff during screening sessions
- Review and understand our Legal Documents, including Terms of Service, Privacy Policy, and Disclaimer
- Promptly notify us of any changes to your contact information
- Direct all medical questions and concerns to your licensed healthcare provider
- Refrain from disruptive, threatening, or abusive behavior
