This Refund Policy governs all purchases, bookings, and transactions with BioX Unlimited LLC, doing business as Function Bone Health and operating Strength Lab Plus wellness studios ("Company," "we," "us," or "our"). By booking our services, creating an account, or making purchases, you agree to the terms of this Refund Policy.
Function Bone Health provides educational wellness screening services and products. All refund terms below apply to these non-medical, educational services.
A security deposit is required for all service bookings. The standard deposit is $50, though deposit amounts may vary by service or package. The deposit secures your appointment time and is applied toward the total cost of your service.
Your deposit will be deducted from the total cost of your service at the time of your appointment. The remaining balance is due at or before the time of service.
If you cancel your appointment at least forty-eight (48) hours before the scheduled appointment time, you will receive a full refund of your deposit to the original payment method.
If you cancel between twenty-four (24) and forty-eight (48) hours before your appointment, fifty percent (50%) of your deposit will be retained as a cancellation fee. The remaining 50% will be refunded to your original payment method or, at our discretion, credited to your account for future use.
If you cancel less than twenty-four (24) hours before your appointment or fail to appear for your appointment without prior notice ("no-show"), your entire deposit is forfeited and is non-refundable. Repeated no-shows (two or more) may result in a requirement for a higher deposit or full pre-payment for future bookings.
You may reschedule your appointment at no additional charge if the request is made at least twenty-four (24) hours before the originally scheduled time. One complimentary reschedule is permitted per booking.
Rescheduling requests made less than twenty-four (24) hours before the appointment are subject to the cancellation policy in Section 3.3 above.
A second or subsequent reschedule of the same booking may result in forfeiture of the deposit. Rescheduled appointments must be completed within sixty (60) days of the original booking date, or the deposit will be forfeited.
Once a screening assessment, consultation, or any other service has been performed and results have been delivered, the service is considered fully rendered and is non-refundable. Screening results are delivered same-day; the service is considered complete upon delivery of results.
If you are dissatisfied with the quality of service you received, please contact us at support@functionbonehealth.com within seven (7) days of your appointment. We will review your concern and may, at our sole discretion, offer a complimentary follow-up session, service credit, or other resolution. This does not constitute a right to a refund for completed services.
Package and bundle pricing reflects a discounted rate conditional on the use of all included services. Individual services within a bundle are non-refundable once rendered.
Unused portions of service bundles will be credited toward future services and are not eligible for cash refund. Bundle credits expire twelve (12) months from the date of purchase.
If you wish to cancel a bundle before all services are rendered, any services already used will be charged at their individual (non-discounted) price, and the remaining balance, if any, will be refunded minus a fifteen percent (15%) administrative fee.
Unopened, undamaged supplement products may be returned within thirty (30) days of the purchase date for a full refund to the original payment method. Products must be in their original, sealed packaging.
Opened supplement products are non-refundable for health and safety reasons, regardless of the amount used.
If you receive a damaged or defective product, contact us within seven (7) days of receipt. We will provide a full replacement or refund at no additional cost. Photographic documentation of the damage may be required.
For product returns, the client is responsible for return shipping costs unless the return is due to a Company error or product defect.
Premium and annual packages are non-refundable after the first service within the package has been rendered.
If you cancel a premium or annual package before any service has been rendered, you may receive a refund minus the deposit and a fifteen percent (15%) administrative fee.
All premium and annual package services must be used within the designated timeframe. Expired, unused services are not eligible for refund.
Promotional credits, gift cards, and referral rewards have no cash value and cannot be redeemed for cash.
Promotional credits are non-transferable and may only be used by the account holder to whom they were issued, unless specifically designated as a gift card.
Promotional credits expire twelve (12) months from the date of issuance unless otherwise specified. Gift cards are subject to applicable state law regarding expiration.
Promotional credits and discounts cannot be combined with other promotions or discounts unless explicitly authorized.
Refunds for payments made with HSA or FSA cards will be returned to the original HSA/FSA payment method. If the original card is no longer active, the refund may be issued via check.
You are solely responsible for any tax implications resulting from HSA/FSA refunds. We recommend consulting your plan administrator or tax advisor regarding the treatment of refunded HSA/FSA expenditures.
Submit refund requests by:
Include your full name, booking confirmation number, date of service, and reason for the refund request.
Approved refunds will be processed within five to ten (5–10) business days and returned to the original payment method. Bank processing times may add additional days before the refund appears in your account.
The Company reserves the right, at its sole discretion, to issue a service credit in lieu of a cash refund where permitted by law.
If you must cancel due to a documented medical emergency, we may, at our sole discretion, offer a full refund or credit regardless of cancellation timing. Documentation from a licensed healthcare provider may be required.
If a service cannot be completed due to equipment malfunction or technical failure on our end, you will receive a full refund or free reschedule at your choice.
In the event of cancellation due to circumstances beyond our reasonable control (natural disasters, pandemics, government orders, etc.), we will offer rescheduling options or account credit. Cash refunds will be considered on a case-by-case basis.
All exception requests are evaluated on a case-by-case basis at the sole discretion of the Company. The Company's decision is final.
Disputes regarding refunds are subject to the Dispute Resolution provisions in our Terms of Service, including mandatory binding arbitration in Naples, Florida (Collier County).
We reserve the right to modify this Refund Policy at any time. Changes become effective upon posting to our Website. The policy in effect at the time of your booking will apply to that booking.
