REMS (Radiofrequency Echographic Multi-Spectrometry) is an FDA-cleared ultrasound technology that measures both bone density and bone quality. A small probe is placed at your lumbar spine and hip to capture ultrasound echo signals. Advanced software analyzes these signals in real time to determine your bone mineral density (BMD), Fragility Score, and 5-year fracture risk — all without any radiation. The entire scan takes approximately 20 minutes.

A European multicenter study of over 4,300 subjects showed REMS achieved 90.4% sensitivity and 95.5% specificity at the femoral neck, and 90.9% sensitivity with 95.1% specificity at the lumbar spine. REMS BMD results correlate closely with DEXA for diagnosing osteoporosis. Importantly, REMS can overcome artifacts like osteoarthritis and vascular calcifications that commonly cause DEXA to overestimate bone density.

DEXA uses low-dose X-rays to measure bone mineral density only. REMS uses ultrasound to measure both bone density AND bone quality (microarchitecture), providing a Fragility Score that DEXA cannot offer. REMS is completely radiation-free, uses portable equipment, can be repeated as often as needed, and does not require a hospital radiology suite. REMS has been shown to be five times more sensitive to BMD changes than DEXA.

REMS is completely radiation-free. It uses the same type of ultrasound technology used in prenatal imaging — no X-rays, no ionizing radiation whatsoever. This means it is safe for all adults, including those who are pregnant, and can be repeated as frequently as needed without any cumulative exposure risk.

Medical guidelines recommend bone density screening for women age 65+, men age 70+, and younger adults with risk factors such as family history of osteoporosis, long-term steroid use, early menopause, smoking, low body weight, or prior fractures. Because REMS is radiation-free, it is also appropriate for younger individuals seeking baseline bone health data and anyone who wants more frequent monitoring than DEXA allows.

A REMS scan is painless and non-invasive. You lie comfortably on an exam table while a technician places a small ultrasound probe on your lower back and hip — similar to a prenatal ultrasound. No needles, no injections, no contrast dye. Preparation is minimal: wear comfortable clothing without metal buttons or zippers near your spine and hips, and empty your bladder beforehand. Avoid calcium supplements 24 hours before the scan.

REMS bone scans at HALLĒ locations typically range from $149 to $299 depending on the assessment package. This is comparable to or lower than out-of-pocket DEXA scan costs ($150–$250). All HALLĒ scans are HSA/FSA eligible. The price includes the scan, a detailed report with T-score, Z-score, Fragility Score, fracture risk assessment, and a same-day consultation to review your results.

Currently, most U.S. insurance plans and Medicare do not specifically cover REMS scans, as a dedicated CPT billing code is in development (expected 2026–2027). HALLĒ accepts HSA and FSA payments and provides detailed documentation you can submit for potential reimbursement. As REMS adoption grows and the billing code is established, insurance coverage is expected to follow.

Because REMS uses zero radiation, there is no limit on scan frequency. You can safely scan multiple times per year to closely track bone health changes. This is a significant advantage over DEXA, which is typically limited to once every 1–2 years due to radiation exposure and insurance restrictions. HALLĒ recommends reassessment every 6–12 months for individuals actively managing bone health.

The Fragility Score is a REMS-exclusive metric that evaluates your bone’s internal microarchitecture — the structural quality that determines how resistant your bones are to fracture. Two people can have identical T-scores (density) but vastly different Fragility Scores (quality). This is why some people with “normal” density still fracture, and others with low density never do. The Fragility Score estimates your personalized 5-year fracture probability, providing information DEXA simply cannot offer.

If you have a total hip replacement, that specific hip cannot be scanned (the same limitation applies to DEXA). However, the opposite hip and lumbar spine can still be assessed. If you have spinal hardware, kyphoplasty, or vertebroplasty, inform your technician so the scan protocol can be adjusted. REMS handles many artifacts better than DEXA because ultrasound is not affected by vascular calcifications or degenerative changes that commonly distort X-ray results.

Astronauts lose bone mass in microgravity, and REMS’s portability, zero radiation, and ability to scan frequently make it ideal for monitoring bone changes in space. This partnership validates REMS as a cutting-edge technology trusted for the most demanding applications.

For many people, yes. REMS provides equivalent bone density measurements plus additional bone quality data that DEXA cannot offer. However, if your physician is actively managing osteoporosis treatment using DEXA-based T-scores as benchmarks, you should discuss incorporating REMS with them. REMS and DEXA measure bone using different technologies (ultrasound vs. X-ray), so direct score comparison between the two is not recommended — each should be compared to its own baseline.
