The Technology
How REMS and DEXA actually work
Answer:DEXA passes two low-dose X-ray beams through bone and calculates density from how much radiation the bone absorbs. REMS sends ultrasound pulses into the same spine and hip sites and analyzes the raw echo spectrum, reading both mineral density and the structural quality of bone — without radiation.

Two X-ray beams at different energy levels pass through your lumbar spine and hip. Because bone and soft tissue absorb the beams differently, the scanner computes your bone mineral density in g/cm², expressed as a T-score against a healthy 30-year-old reference.
It measures the image of your bone — which is exactly why anything that changes the image (arthritis, calcified vessels, compressed vertebrae, metal) changes the number.
An ultrasound probe at the same spine and hip sites captures raw, unfiltered echo signals. Software compares their full radiofrequency spectrum against reference models, producing the same T-score and Z-score as DEXA — plus a Fragility Score for bone quality.
It measures the tissue itself rather than an image of it, so the artifacts that distort X-ray densitometry are largely discarded in the signal analysis.

Why Precision Matters
Your bones deserve better than an average
Answer:Precision decides how soon you can trust a change in your bones. REMS reproduces its own measurement to within roughly 0.4%, while a repeat DEXA can vary enough that small changes may be measurement noise. Higher precision means real change shows sooner — which makes personalized, data-driven prevention practical.
A population statistic is not a plan
A T-score compares your bone to an average 30-year-old’s. That is a useful screening convention, but it flattens you into a percentile. Two women with the identical T-score of −1.8 can carry very different fracture risks — one with dense-but-brittle bone, one with less-dense-but-well-structured bone. Density alone cannot tell them apart; in a 5-year study of 1,989 adults, the REMS Fragility Score separated who fractured from who did not better than either technology’s T-score. Personalization starts with measuring the individual, not ranking them.
Noise is the enemy of feedback
Bone changes slowly — a good year of training and nutrition might move density a few percent. If your instrument’s scan-to-scan variability is of the same size as the change you are trying to detect, you are flying blind: an encouraging result may be noise, a discouraging one may be noise too. This is why DEXA guidelines require each facility to calculate its own “least significant change” before trend claims are made, and why comparisons across different DEXA machines are unreliable. REMS’s published precision error of 0.32–0.38% leaves room for a real signal to emerge above the noise floor — the difference between guessing and knowing whether your protocol works.
Measurement you can repeat becomes a feedback loop
A scan you take once a decade is a verdict. A scan you can repeat — safely, affordably, without a dose ceiling — is a feedback loop: baseline, intervene, re-measure, adjust. That loop is the entire logic of personalized prevention, and radiation-free measurement is what makes it possible. It is the difference between being told at 68 that you have osteoporosis and watching your own trajectory from 45 — while there is still time to bend the curve.
The Honest Part
Where DEXA still wins
Answer:DEXA remains the reference standard for diagnosing osteoporosis and the measurement treatment guidelines are written around. Insurance, including Medicare, covers it; prescribing thresholds use its T-scores; and decades of outcome data stand behind it. If medication or insurance authorization is on the table, you will need a DEXA scan.
It is the standard your treatment will be judged against
FRAX inputs, medication thresholds, and monitoring protocols are built on DEXA-derived BMD. A REMS report informs the conversation; it does not currently substitute in those pathways.
Insurance covers it
Medicare covers DEXA for eligible adults, and most insurers follow. REMS is typically self-pay. If cost is the deciding factor and you qualify, DEXA is the rational first scan.
Its evidence base is decades deep
Every major osteoporosis drug trial used DEXA endpoints. REMS validation is strong and growing — roughly a decade of published studies — but it cannot yet match that historical depth.
Specific populations are better characterized on DEXA
In disuse-related osteoporosis (for example after spinal cord injury), an independent study found poor REMS–DEXA concordance. Honest limits are limits: for these patients, DEXA remains the better-understood tool.

Choose DEXA when insurance coverage or medication qualification drives the decision. Many people benefit from both: DEXA for the medical record, REMS for the fuller, more frequent picture.

For measuring bone density, yes — within well-studied limits. In a European multicenter study of 4,307 women, REMS identified osteoporosis with 90.4% sensitivity and 95.5% specificity at the femoral neck compared against DEXA, with similar results at the spine. REMS also reproduces its own measurement more tightly (0.32–0.38% precision error). One independent study found lower diagnostic concordance in disuse-related bone loss, so DEXA remains preferred in that specific population.
Not for every purpose. If your physician needs a scan to qualify you for osteoporosis medication, or your insurer requires DEXA for treatment monitoring, you will still need a DEXA — prescribing guidelines and coverage rules are written around it. REMS works alongside DEXA: it adds a bone-quality reading (Fragility Score), removes the radiation constraint on repeat scanning, and provides a second opinion when a DEXA result looks inconsistent with your history.
Two common reasons. First, artifacts: spinal arthritis, bone spurs, and aortic calcification all add apparent density to a DEXA image, so a fragile spine can read as normal. Second, density is not the whole story — fracture resistance also depends on bone microarchitecture, which standard DEXA does not measure. In one study of women with artifact-inflated DEXA readings, REMS reclassified 58.7% of previously fractured patients as osteoporotic, versus 23.3% by DEXA.
The Fragility Score is a 0–100 index of bone structural quality, computed from the raw ultrasound spectrum during a REMS scan — lower is better. In a 5-year study of 1,989 adults, it predicted incident osteoporotic fractures with an AUC of 0.811 in women and 0.780 in men, outperforming both the REMS and DEXA T-scores in the same cohort. It answers a different question than density: not how much bone you have, but how well that bone is likely to hold.
No. REMS uses the same class of ultrasound as prenatal imaging — no X-rays and no ionizing radiation. That is why it can be repeated as often as clinically useful, and why it is an option during pregnancy, when DEXA is not performed. A DEXA scan uses a low X-ray dose; the dose is small, but it is one reason DEXA scans are typically spaced 1–2 years apart.
Increasingly, yes. REMS reports T-scores and Z-scores using the same WHO diagnostic thresholds as DEXA, US practice parameters for REMS evaluations were published in 2025, and the technology is FDA-cleared. Many physicians use REMS results as complementary information alongside DEXA. Bring your full REMS report to your appointment — and note that for insurance-mandated treatment decisions, your physician may still order a DEXA.
DEXA monitoring is typically spaced 12–24 months apart — partly because of the radiation dose, and partly because its scan-to-scan variability means small changes cannot be distinguished from measurement noise sooner. REMS carries no radiation dose and reproduces its measurement to within roughly 0.4%, so the re-scan interval becomes a clinical decision rather than a safety constraint. Many people tracking an intervention re-scan every 6–12 months.
DEXA is usually the cheaper option out of pocket if you qualify for coverage: Medicare and most insurers cover it for women 65+, men 70+, and younger adults with risk factors.
